Incident Narrative

An employee was using a crowbar to lift a powered industrial truck outrigger to dislodge debris from the metal outrigger wheel. While he was using his left hand to brush the debris away from the wheel, the crowbar slipped and allowed the wheel to fall on his left hand, amputating his middle fingertip and breaking his pointer finger.
