Incident Narrative

An employee was climbing a steel fixed ladder that led to a steel-framed basket attached to a crane. As he was trying to grasp the top rung of the ladder, he lost his grip and fell backward approximately 12.5 feet to the ground below, landing on his right side and fracturing his pelvis.
