Incident Narrative

An employee was connecting a temporary 480 Volt AC electrical pigtail cord into an electrical box. This pigtail would supply the plug connection for the 480 Volt AC cord for the floor shot blasting machine. An arc flash occurred burning the employees hands and requiring hospitalization.
