Incident Narrative
An employee was measuring duct for insulation while standing on a sheet of plywood that had been laid across the beams of the ceiling. The end of the platform was extending over a beam, so when the employee stepped on the end of the plywood he fell approximately 8 feet. He fractured seven ribs and was hospitalized overnight.
