Incident Narrative
The employee was working as a shackle chuck. He places a shackle on the beef quarter so that it can be moved to a different department. He noticed some gloves on top of a rotating sprocket above his work area. He reached his right arm to grab the gloves and his sleeve was caught in the chain, drawing his right arm into the sprocket and fracturing the radius and ulna.
