Incident Narrative
On Friday, October 7, 2016, two employees were helping to decontaminate the blow down settling drum in the coker unit. In order to remove the check valve, one employee had to hold the back up while the other loosened the check valve. As the employee began to loosen the check valve, it suddenly came off and product sprayed out of the line onto the two employees. As a result, one employee received burns to his face and hands, requiring hospitalization.
