Incident Narrative
An employee was attempting to fuel his truck with diesel. He removed the fuel cap from his truck, picked up the fuel nozzle, placed the fuel nozzle in the tank, and squeezed the handle. It was at that point that a flash fire and/or small explosion occurred, burning his face, hands, and chest area. He was hospitalized for third degree burns.
