Incident Narrative
At approximately 2:40 a.m. on October 27, 2016, the injured employee and other employees were removing an 80 lb. bolt from a stamping press. The bolt had to be moved approximately 2 ft. over the edge of the press. The bolt was attached to a chain fall and sitting on the lip or edge of the press. An employee pulled the bolt with the tag line, but could not move the bolt over the edge. The injured employee then pushed the bolt with his hands. The bolt moved, and his right hand or finger was caught between the bolt and the edge. The employee suffered a crush/dislocation injury and his right pinky fingertip was medically amputated below the fingernail.
