Incident Narrative

An employee was working on equipment that was not in operation. A chain on the equipment was bound up and had tension on it. The employee took a bar to pry and loosen the chain because the chain had some energy in it. When the chain became free, the chain moved slightly, energized. His finger was above the chain and it slipped between the sprocket and the chain. His right index fingertip was caught between the chain and the sprocket and amputated.
