Incident Narrative

An employee climbed a 12-foot step ladder. He stepped off the ladder with his left foot to position it on a large round valve handle to reach an anchor point he intended to tie off to. As he reached with his left hand, to attach his lanyard to this anchor point, the large round valve turned, and the employee lost his balance and fell approximately 6 feet onto a pipe below the ladder. The employee sustained a fractured left arm, fractured thumb and two fractured ribs, requiring hospitalization.
