Incident Narrative

On 09/30/2016, at 7:00 PM, a shipping/receiving employee was preparing to shrink wrap a switchboard when she stepped over the shrink wrap placed between the film roll and the switchboard structure to start the turntable wrapper. While stepping over it, her right foot was caught on the film and she fell to the ground on her left side, fracturing her left hip.
