Incident Narrative
An employee and two coworkers were reverse-rolling a 110-foot roll of carpet for a scheduled carpet installation. After reverse-rolling the carpet, the team proceeded to operate the carpet machine in order to shrink-wrap it. During this process, the carpet fell off the machine and struck the injured employee in the left leg, breaking the kneecap and requiring surgery.
