Incident Narrative

On September 29, 2016, an employee was helping to install a torque (transmission) on a large bulldozer using chain hoists when one of the hoists gave way, allowing the transmission to come down and strike the employee's head and face. The employee suffered forehead lacerations, facial trauma, and a skull fracture.
