Incident Narrative

An employee was in the attic of a residential home setting up equipment to blow insulation into the attic. He was walking on the ceiling joist while pulling an insulation hose when he lost his balance and his foot slipped off the joist. One of his feet went through the ceiling and caused his upper torso to fall onto the joist, fracturing four ribs and bruising his chest.
