Incident Narrative
An employee was monitoring the production conveyor to remove defective products from production. While he was pulling a large piece of plastic off the conveyor, the piece of plastic fell into the equipment. The plastic started making a loud noise, so the employee attempted to remove it. His hand was then pulled into the equipment's wheel area. His right thumb and index finger had to be partially amputated. The guard was in place at the time.
