Incident Narrative

While working in a ditch, an employee cut a test header off a piece of steel pipe. The test header was being lifted out of the ditch when it tipped to the side and some methanol, which had been used to clean the pipe, leaked out and onto the employee's legs. The methanol ignited and caught the employee's legs on fire. The employee was hospitalized with leg burns.
