Incident Narrative

An employee entered a basket/cage that was on a forklift to secure a load to a trailer. The forklift operator lifted the employee in the basket to the top of the load. The basket became hung up on the load, so the forklift operator backed up the forklift, which caused the employee and the basket to drop to the ground. The employee suffered a fractured face, requiring stitches, and a fractured right wrist.
