Incident Narrative

An employee at a utility power pole was moving an insulator cover that was draped over a power wire. He was working from a bucket truck about 30 feet up. When the employee removed the insulator, the insulator cover failed and an arc flash occurred, causing flash burns to the employee's hands. He was hospitalized.
