Incident Narrative

An employee was supervising as a forklift operator was unloading switchgear. While retracting the forks, the forklift operator backed into a lip on the ground, which caused the forks to lift up and the switchgear to tip over. The switchgear fell onto the employee's right leg, breaking two bones.
