Incident Narrative

An employee was moving a portable bagging conveyor when the legs folded, causing the employee to fall onto the conveyor. The employee's left- and right-hand fingers were then caught between the legs of the conveyor and the conveyor itself. The left ring and middle fingers were amputated to the first knuckle, and the right index finger required stitches.
