Incident Narrative

An employee was cutting a concrete form on a window sill using an angle grinder with a cut-off blade. The grinder went through the plywood and struck the concrete, resulting in a kickback that caused the grinder to strike and deeply lacerate the employee's chest. He was hospitalized.
