Incident Narrative

An employee was tightening packing on a load pump with the truck pump drive shaft engaged. The employee's clothing became entangled in the drive shaft and pulled the employee's arms in resulting in multiple fractures to the left arm, skin abrasions on the left hand, and bruising to the right arm.
