Incident Narrative
An employee was changing out a band saw blade. The employee loosened the wheel for the saw and went to get a forklift to lift and remove the wheel from machine. When he went to put chains on the wheel, the wheel fell and hit the door of the band saw machine causing it to swing open and strike him in the head resulting in a contusion to the forehead.
