Incident Narrative

An employee was changing a roll on a packaging machine when a roll of paper became hung up on a frame. The employee attempted to free it by pushing on the roll. When it came free, the employee's right index finger was caught between the frame and the roll of paper, amputating it at the first digit just behind the nail.
