Incident Narrative

An employee was removing plastic inserts from the back side of a precast panel. He was standing on a trailer, which had the precast wall on the bed. When he stepped out, he lost his footing and fell 4 feet to the ground on his right side, head and shoulder first. He fractured some right-side ribs and his collarbone, and his right lung collapsed.
