Incident Narrative

An employee was using a 2-inch screw pipe to remove a coupler. He used a pipe threading machine and pipe wrench to force the coupler to screw off. His right ring finger was pinched between the pipe wrench and the pipe threading machine, amputating his distal fingertip.
