Incident Narrative

An employee was taking laser measurements at a client facility in the freezer area. There was a roof leak and patches of black ice on the floor. The employee slipped on the ice and fell to the floor, sustaining a hairline fracture to his right hip and injuring his right arm. He was hospitalized.
