Incident Narrative

An employee was moving pallets with an electric pallet jack. While backing up the pallet jack, his leg got caught in the wheel well of a forklift. The forklift operator began to move the forklift forward, causing the employee's leg to be pulled farther into the wheel well. He suffered broken bones in his right foot and a fractured right elbow, requiring hospitalization.
