Incident Narrative
An employee was attempting to manually unjam the pleating machine. The stripper bar broke free of the pusher bar and cycled down to the table before returning to and resting in the up position. The employee's left hand was caught between the stripper bar and table during the down cycling of the bar. His left hand was sliced between the index and middle fingers to the wrist, and half of his hand was amputated.
