Incident Narrative

An employee was supervising the loading of a coil onto a truck from the designated safe zone when the 500-foot, 4-inch coil weighing approximately 1,250 pounds began to slip off the spool that was being lifted by a forklift. The employee tried to push the coil back onto the spool when the coil struck him, knocking him to the ground and cracking his vertebrae.
