Incident Narrative
An employee was trying to install a plug on a blender conveyor to stop meat trimmings from going into the blender. Her fingers then came in contact with the conveyor paddle of the conveyor, amputating a portion of the third finger (from the fingernail to the tip) and causing an avulsion to the second finger.
