Incident Narrative

An employee had finished rolling and wrapping a deck sheet and was using a hoist with hooks to lift it when he noticed a problem and grabbed the hook. At the same time, the deck sheet came out of the hook on his side. His finger became wedged between the air bar with the rolled deck sheet and the bracket that holds the air bar on the power roller, partially amputating his left ring fingertip.
