Incident Narrative

An employee was removing a tire from a forklift. While removing the tire from the split rim, the employee removed all of the split rim bolts. When the tire was inflated, the split rim separated and struck the employee in the left side of his head, face, and jaw. He suffered lacerations that required surgery.
