On or about July 7, 2016, at 5:00 p.m., an employee fell down the marble staircase as she was leaving for the day. She was hospitalized with a lacerated bottom lip, loose teeth, dislocated right elbow, fractured right ulna and radius, concussion, and multiple bruises.
