Incident Narrative
An employee was performing duct work demolition from an extension ladder approximately 15 feet off the ground. The ladder feet slipped and the employee fell from the ladder, which then fell onto the employee. The employee suffered a dislocated shoulder and head/arm lacerations, requiring hospitalization.
