Incident Narrative

An employee was performing maintenance on skid steer equipment. The employee was changing the bucket and putting pins on. The lock pins were stuck so the employee used a pry bar to pop it loose, but it did not come apart. The employee asked another worker to tilt the skid while the pry bar was in the front of the equipment. The employee's left ring finger got caught between the pry bar and the skid steer carriage resulting in amputation of the finger.
