Incident Narrative

An employee was pulling the die from a rack when the push/pull mechanism of the die cart failed. When the employee realized this, he removed the chains from around the die and proceeded to set the brake, then pushed the die onto the die cart from behind the rack. The die cart then moved backwards, causing the die to start to tip and fall. When the employee tried to grab and stabilize it, the center of gravity for the die was over the edge of the die rack shelf, and the die fell and trapped the employee's arms between the top of the die and the underside of the die rack shelf. He broke both wrists and was hospitalized.
