Incident Narrative
An employee was packaging PVC pipe on an extruder when the PVC pipe became misaligned on the transfer arms prior to the chamfering process. When the employee attempted to realign the pipe, the chamfer unit activated, causing the employee's thumb to become trapped between the pipe and the chamfer device. The employee's left thumb was partially amputated.
