Incident Narrative

On June 17, 2016, an employee was troubleshooting an aerosol line can labeler to try and release a jam. The machine was shut off and did not restart. While investigating the drive belt, his middle and ring fingers were mashed between the belt drive and belt pulley. His fingertips had to be surgically amputated.
