Incident Narrative

An employee was hitting a refractory block that had wedged between a wall and a floor with a sledge hammer when another refractory block fell and struck him. Each block weighs over 200 pounds. The employee's right shoulder was injured. The employee also suffered second degree burns and was hospitalized.
