Incident Narrative

An employee was removing burrs on a set of custom-made aluminum jaws with a 10-inch hand file, when the file struck the jaws while they were still spinning in the chuck of the lathe. The file then pushed back into the employee's left-hand palm, causing a deep puncture wound.
