Incident Narrative
Employees were taking a delivery of long pipe from the loading dock to the hoist and then to the eighth floor using a dolly. As they were coming down the ramp off the hoist, the dolly began to tip over and one of the pipes struck an employee's foot. The employee was hospitalized with a broken foot.
