Incident Narrative
On or about 6/6/2016, at approximately 0900, an employee was walking in an aisle way when he was struck from behind by a forklift carrying a steel coil. The forklift operator stopped the forklift, causing the steel coil to fall and contact the employee's foot, amputating his left toes.
