Incident Narrative
An employee was taking scaffolding down when the chain hit the safety on the hook. The safety would not hold, and the chain could not handle the load. The employee pulled his hand away and the chain nipped his fingers, amputating his left pinky fingertip and fracturing his left ring finger.
