Incident Narrative

An employee was bending rebar and turned the switch to the program position so he could check the angle of the bar. The bending pin went to the left side of the bar. As he turned the equipment back on in the manual mode, the bending pin returned to the home position. The employee had already loaded a second bar. When the bending pin was returning to the home position, it picked up the second bar. As the employee reached for the bar, his left index finger was caught between the pin and rebar, amputating the first digit.
