Incident Narrative

An employee was using an overhead hoist with a small J-hook to lift a raw part up from a work table in order to change the orientation from face-down to face-up. He had lifted the part approximately 3 inches from the table surface when the part became unsecured from the hook and dropped on his left hand. The fifth digit was smashed and required stitches, and the fourth digit was partially amputated at the first distal.
