Incident Narrative
On May 10, 2016, at approximately 3:30 p.m., an employee was observing the trim saw during a mill shutdown. He was standing on a 4-foot-wide catwalk, located on the green chain side of the trim saw, when he lifted a top-hinged steel door to gain access to the saws. Before he could clamp the door open, the door slipped and came down, striking his head. He was knocked over the 36- to 42-inch railing and fell 12 feet to the concrete floor below. The employee was hospitalized for fractures to the wrist, eye socket, and kneecap.
