Incident Narrative

On May 9, 2016, at about 4:00PM an employee was admitted to the hospital due to a forearm laceration with a circular saw while cutting a piece of wood. On May 9, 2016, at about 4:30PM another employee was injured when a forklift struck the concrete structure the employee was working on, tilting the structure, and pinching the employee's left foot under it. The employee was admitted to the hospital due to the injury on his foot. Note to user: This record contains two separate events that were reported simultaneously to OSHA by the employer.
