Incident Narrative
An employee was operating the pallet machine, which prepares pallets of rice for transport. A bag of rice fell off the pallet while being transported via rollers to where the pallets are wrapped in plastic. The employee went to grab the bag and his right glove was caught between the rollers, amputating his right index finger pad.
