Incident Narrative
Employees were measuring the receiving dock bay door area for a replacement dock plate. The dock plate springs opened quickly and the dock plate caught the injured employee's fingers in a pinch point between the dock plate and the dock bay door surface resulting in the amputation of the right middle and index fingertips.
